Approval Process for all Academy Volunteers, Tutors,  Host Families and Volunteer Drivers

In compliance with Massachusetts law, Phillips Academy obtains criminal offender record information (CORI) for every prospective employee and volunteer who may have direct and unmonitored contact with children.

Parents and others interested in volunteering, as well as host family members over the age of 21, must complete and sign the following two forms:

  • Criminal Offender Records Information request form to authorize the academy to conduct a CORI check; and
  • Academy Requirements for Volunteer Drivers form to acknowledge that you understand the approval process for volunteer drivers, even if you do not wish to serve as a volunteer driver.  Parents of Students of Phillips Academy (PSPA) requested that the latter document be posted on the Web site so parents who are interested in driving to support the academic, community services or athletic program know the requirements to do so. Please note:  If you are interested in serving as a volunteer driver, you must also submit your driving record and proof of insurance, as well as participate in the mandatory driver training as outlined in this form.

Tutors hired by parents to work on campus, who are paid by parents, must complete and sign the:

Approved tutors may work with students in the basement of the Oliver Wendell Holmes Library, and will be required to sign in and out at the front desk.  Tutors hired by students and their families may not have dorm access.

Please send signed CORI and/or Volunteer Driver forms and supporting documentation as required to:

The human resources office will process the request and upon receiving the record from the Commonwealth will report to each volunteer or tutor whether he or she is approved.
